What documents should I submit?
When submitting an application for SHIP Homeowner Rehabilitation Assistance, all of these documents must be provided as applicable for all household members over the age of 18 unless otherwise noted.
• Copies of last 30 days pay stubs.
• Copy of last two years W-2 statements.
• Copies of Social Security Benefits Award letters or pension letters.
• Copies of the complete income tax return form the last two years.
• Copies of bank statements for the last six months.
• Copies of investment statements from the last six months.
• For self-employed applicants or household members, copies of two years corporate tax returns and a profit and loss statement from the preVious three months.
• Copy of court order if receiving child support.
• Proof of AFDC or other type of assistance being received.
• Copy of recorded House Warranty Deed.
• Copy of current mortgage statement indicating remaining principal amount and monthly payment.
• Copy of most recent tax bill
• Proof of any other type of miscellaneous income received.
• Copy of social security cards for all household members.
• Copy of Florida Driver's License or Florida ID card.
